1. Scope of Services

 

We provide:

• Relaxation Massage (stress relief, light to moderate pressure)

• Therapeutic Massage (pain relief, deep tissue, corrective bodywork)

Therapeutic massage service are provided for the purpose of stress reduction, pain relief, muscle tension reduction, and improvement of circulation and mobility. Services may include but are not limited to deep tissue massage, manual stretching, cupping therapy, and hydrotherapy applications (Hot and cold compress applications).

 

Massage therapy is not a substitute for medical examination, diagnosis, or treatment.

 

2. Client Health Disclosure

Clients are required to:

• Complete a health intake form prior to receiving services.

• Disclose all medical conditions, injuries, surgeries, medications, and contraindications.

• Inform the therapist of any changes in health status.

Failure to disclose relevant health information releases Underground Spa and Wellness LLC from liability related to undisclosed conditions.

3. Treatment Plan Recommendations

Treatment plans are developed based on professional assessment and client goals. recommended frequency and duration of sessions are suggestions only.

Results vary depending on:

• Consistency of sessions

• Client compliance with home care recommendations

• Overall health condition

No guarantees of specific outcomes are made.

4. Payment and Package Policy

• Payment is due at time of service unless prepaid as part of a package.

• Prepaid packages are non-refundable and non-transferable.

• Packages expire within the stated time period from purchase date.

• No refunds will be issued for partially used plans.

 

5. Cancellation & No-Show Policy

• A minimum of 24-hour notice is required for cancellation or rescheduling.

• Late cancellations or no-shows may result in full session charge or loss of session.

• Repeated missed appointments may result in termination of services.

 

6. Professional & Legal Compliance

All services are provided within the scope of practice for a Licensed Massage Therapist under Maryland law. No medical diagnosis, prescription, or chiropractic adjustment are performed.

7. Liability Waiver

I understand that massage therapy involves manipulation of soft tissues. While every effort is made to ensure safety and comfort, I assume responsibility for any known or unknown risks.

I release and hold harmless Underground Spa and Wellness LLC from liability arising from:

• Failure to disclose medical conditions

• Failure to follow post-treatment care instructions

• Conditions outside the therapist"s scope of practice.

 

8. Right to Refuse Service

Underground Spa and Wellness LLC reserve the right to refuse or discontinue service if treatment is deemed unsafe, inappropriate, or outside professional boundaries.

Membership Terms

• Membership is billed monthly

• 30-day written notice is required for cancellation.

• sessions are non-refundable and non-transferable.

• Unused sessions roll over for one month only.

• Missed sessions without 24-hour notice may be forfeited.

All services are strictly professional and therapeutic in nature. Any inappropriate behavior will result in immediate termination of the session without refund.
